World News in Brief: 1.3 billion teens suffering mental disorders, Russia’s Indigenous Peoples face ‘extinction’, Belarus rights update

At least one in seven youngsters has a mental disorder. That’s the warning from the UN World Health Organization (WHO), which has appealed for much greater investment to help almost 1.3 billion teenagers globally.

WHO Director-General Tedros Adhanom Ghebreyesus said that failing to address the mental, sexual and reproductive health of adolescents will have “serious and life-threatening consequences for young people”. It will also come at a massive cost for society, which justifies a major public investment from governments worldwide.

Tedros noted that anaemia among adolescent girls remains “prevalent” and at levels similar to those in 2010, while close to one in 10 teenagers is obese.

STDs on the rise

Sexually transmitted infections (STIs) including syphilis, chlamydia, trichomoniasis and genital herpes that commonly occur among youth are on the rise too.

If left untreated, they could have “lifelong implications for health”, the WHO chief said, citing new data.

Tedros also spoke out against attempts to “roll back” young people’s access to sexual and reproductive healthcare and sex education in response to growing opposition to gender equality and human rights.

He said that any restrictive age of consent policies limit young people’s access to critical services, including those for sexually transmitted infections and HIV.

Adolescence is a unique and critical stage of human development, involving major physical, emotional and social transitions, and is a pivotal window for laying long-term foundations for good health, WHO notes.

“Promoting and protecting the health and rights of young people is essential to building a better future for our world,” said Tedros.

“Conversely, failing to address the health threats that adolescents face – some longstanding, some emerging – will not only have serious and life-threatening consequences for young people themselves, but will create spiraling economic costs.”

Gains are possible

The publication was launched at an event on the margins of the UN Summit of the Future .

“Adolescents are powerful and incredibly creative forces for good when they are able to shape the agenda for their wellbeing and their future,” said Rajat Khosla, executive director of the Partnership for Maternal, Newborn and Child Health, which co-hosted the launch.

“Leaders must listen to what young people want and ensure they are active partners and decision makers,” she added.

Indigenous Peoples of Russia face ‘extinction’ from Ukraine mobilisation

Russia’s Indigenous Peoples face “extinction” because they have been subjected to “massive” mobilisation to fight in the war in Ukraine, a top independent rights expert said on Monday.

The Special Rapporteur on the situation of human rights in Russia, Mariana Katzarova, maintained that most of the mobilisation of minority communities had been forced.

“The mobilisation of Indigenous Peoples, particularly from small-numbered nations, is massive, and the death rate is massive, which is threatening them with extinction,” she said, citing civil society data.

‘Almost no Slavic faces’ on frontline

The independent rights expert, who does not work for the UN or receive a salary from the Organization, said that following the full-scale Russian invasion of Ukraine in February 2022, she saw “almost no Slavic faces” on images broadcast from the frontline, but rather those of Russia’s ethnic peoples.

“It was the Buryatians, it was the Kalmykians, it was the Chechens, it was the national minorities of Russia,” she insisted.

Speaking in Geneva, Ms. Katzarova said that Russian authorities had gone to the country’s “faraway places” to find war recruits.

“The mobilisation hasn’t been so brutal in Moscow and St. Petersburg…It’s the most sophisticated places where people know their rights.

“But, when you go 100 miles away on the train from Moscow and St. Petersburg and let alone in the far-away regions of Siberia…people don’t even feel they have a choice. They don’t even know their rights.”

The independent rights expert said she had documented cases where the military had gone “door to door” in their search for soldiers and “just drag out the men from Indigenous villages”.

Ms. Katzarova is due to present her report to the Human Rights Council on Tuesday .

Belarus authorities quell ‘all avenues of dissent’, UN Rights Council hears

Serious rights violations are continuing against civil society and critics of the Government in Belarus, the UN Human Rights Council heard on Monday.

Appointed by the Council in Geneva, the Group of Independent Experts on Belarus highlighted numerous grave abuses linked to protests at the disputed re-election of Presidential Alexander Lukashenko in 2020.

These include deaths, torture, gender-based violence and denial of the right to a fair trial, said Karinna Moskalenko, chair of panel of independent experts.

Climate of fear

She said the government “continues to instill a pervasive climate of fear by quelling all avenues of dissent, including in the digital space. New electronic intelligence equipment appears to have been pursued to increase the monitoring of online activities, ahead of the Presidential elections.”

Ms. Moskalenko, who as an independent expert does not work for the UN, further maintained that President Lukashenko’s government was “responsible for the near-total destruction of civic space and fundamental freedoms in Belarus”.

Most opponents of the authorities had been “either imprisoned or forced into exile since the 2020 elections”, she noted.

NIH is mandated by the Public Health Service Act sec. 492B, 42 U.S.C. sec. 289a-2 to ensure the inclusion of women and members of racial and ethnic minority groups in all NIH-funded clinical research in a manner that is appropriate to the scientific question under study. The primary goal of this law is to ensure that research findings can be generalizable to the entire population. Additionally, the statute requires clinical trials to be designed to analyze whether study outcomes differ for women and members of racial and ethnic minority groups.

Implementation

Applications & proposals.

All NIH-funded studies that meet the NIH definition for clinical research must address plans for the inclusion of women and minorities within the application or proposal. Using the PHS Human Subjects and Clinical Trial Information Form, applications and proposals should describe the composition of the proposed study population in terms of sex or gender, racial, and ethnic groups, and provide a rationale for the proposed section. Any exclusions based on sex or gender, race, or ethnicity must include a rationale and justification based on a scientific or ethical basis. Investigators should also plan for appropriate outreach programs and activities to recruit and retain the proposed study population consistent with the purposes of the research project. Refer to the PHS Human Subjects and Clinical Trial Information Form Instructions for complete guidance on what to address in your application.

Peer Review

Scientific Review Groups will assess each application/proposal as being "acceptable" or "unacceptable" with regard to the inclusion of racial and ethnic minorities and women in the research project. For additional information on review considerations, refer to the Guidelines for the Review of Inclusion in Clinical Research . For information regarding the coding used to rate inclusion during peer review, see the list of NIH Peer Review Inclusion Codes .

Progress Reports

NIH recipients/offerors must collect and annually report information on sex or gender race, and ethnicity in progress reports. Refer to this Decision Tree for help determining reporting expectations for different types of studies.

Special Considerations for NIH-defined Phase III Clinical Trials

Applications & Proposals: If the proposed research includes an NIH-defined Phase III Clinical Trial , evidence must be reviewed to show whether or not clinically important differences in the intervention effect by sex or gender, race, and/or ethnicity are to be expected. The application or proposal must address plans for the valid analysis of group differences on the basis of sex or gender, race, and ethnicity unless there is clear evidence that such differences are unlikely to be seen.

Progress Reports: For projects involving NIH-defined Phase III Clinical Trials, annual Research Performance Progress Reports (RPPRs) should include a statement indicating the status of analyses of the primary outcome by sex or gender, race, and ethnicity. The results of these analyses should be included in the “Project Outcomes” section of the RPPR. See the Sample Project Outcomes page for an example.

Registering & Reporting in ClinicalTrials.gov: NIH-defined Phase III Clinical Trials that also meet the definition of an applicable clinical trial must report the results of the valid analysis of group differences in ClinicalTrials.gov. The valid analyses should be done for each primary outcome measure by sex or gender, and race and/or ethnicity. Upon study registration in ClinicalTrials.gov, outcome measures should be pre-specified by sex or gender, and race and/or ethnicity to prepare for reporting results in this stratified manner. Refer to the Guidance for Valid Analysis Reporting and NOT-OD-18-014 for additional information.

Gunman who killed 10 at Colorado supermarket found guilty, faces life in prison

BOULDER, Colo. (AP) — A mentally ill man who killed 10 people at a Colorado supermarket in 2021 was convicted Monday of murder and faces life in prison.

Defense attorneys did not dispute that Ahmad Alissa, who has schizophrenia, fatally shot 10 people including a police officer in the college town of Boulder. But he pleaded not guilty by reason of insanity, with the defense arguing he couldn’t tell right from wrong at the time of the attack.

In addition to 10 counts of first-degree murder, the jury found Alissa guilty on 38 charges of attempted murder, one count of assault, and six counts of possessing illegal, large-capacity magazines.

Alissa did not visibly react as the judge began reciting the guilty verdicts against him. He sat at a table with his attorneys and appeared to trade notes with members of the defense team, speaking quietly at times with one of his attorneys.

Judge Ingrid Bakke had warned against any outbursts. There were some tears and restrained crying on the victims’ side of the courtroom as the murder convictions were read.

The courtroom was packed largely with victims’ families and police officers, including those who were shot at by Alissa. Several members of Alissa’s family sat just behind him.

Alissa started shooting immediately after getting out of his car in a King Soopers store parking lot in March 2021. He killed most of the victims in just over a minute and surrendered after an officer shot him in the leg.

Prosecutors had to prove Alissa was sane. They argued he didn’t fire randomly and showed an ability to make decisions by pursuing people who were running and trying to hide from him. He twice passed by a 91-year-old man who continued to shop, unaware of the shooting.

He came armed with steel-piercing bullets and illegal magazines that can hold 30 rounds of ammunition, which prosecutors said showed he took deliberate steps to make the attack as deadly as possible.

Several members of Alissa’s family, who immigrated to the United States from Syria, testified that he had become withdrawn and spoke less a few years before the shooting. He later began acting paranoid and showed signs of hearing voices, they said, and his condition worsened after he got COVID-19 in late 2020.

Alissa was diagnosed with schizophrenia after the attack and experts said the behaviors described by relatives are consistent with the onset of the disease.

State forensic psychologists who evaluated Alissa concluded he was sane during the shooting. The defense did not have to provide any evidence in the case and did not present any experts to say that Alissa was insane.

Despite the fact that he heard voices, the state psychologists said, Alissa did not experience delusions. They said his fear that he could be jailed or killed by police revealed Alissa knew his actions were wrong.

Alissa repeatedly told the psychologists that he heard voices, including “killing voices” right before the shooting. But Alissa failed during about six hours of interviews to provide more details about the voices or whether they were saying anything specific, forensic psychologist B. Thomas Gray testified.

The defense pointed out that Gray and and his partner, Loandra Torres, did not have full confidence in their sanity finding, largely because Alissa did not provide more information about his experiences even though that could have helped his case. Gray and Torres also said the voices played a role in the attack and they didn’t believe it would have happened if Alissa were not mentally ill.

Mental illness is not the same thing as insanity. Colorado law defines insanity as having a mental disease so severe that it’s impossible for a person to tell right from wrong.

Family members of the victims attended the two-week trial and watched graphic surveillance and police body camera video. Survivors testified about how they fled and in some cases helped others to safety.

Prosecutors did not offer any motive for the shooting. Alissa initially searched online for public places to attack in Boulder, including bars and restaurants, then a day before the shooting focused his research on large stores.

On the day of the attack, he drove from his home in the Denver suburb of Arvada and pulled into the first supermarket in Boulder that he encountered. He shot three victims in the parking lot before entering the store.

An emergency room doctor said she crawled onto a shelf and hid among bags of potato chips. A pharmacist who took cover testified that she heard Alissa say “This is fun” at least three times as he went through the store firing his semi-automatic pistol that resembled an AR-15 rifle.

Alissa’s mother told the court that she thought her son was “sick.” His father testified that he thought Alissa was possessed by a djin, or evil spirit, but did not seek any treatment for his son because it would have been shameful for the family.
